  • With more than 40 years of experience, we are America’s leader in home care services.

Gentiva is now an affiliate of Kindred at Home

By expanding our Kindred at Home nationwide reach of home health and hospice services, we can provide the care that our patients deserve in the place they want it the most – home.

Home Care – A Bridge Between Your Doctor and Home

Our clinicians bring state-of-the-art care home. We assess your environment, needs and goals and work with you to achieve the results you want. We also coordinate with your doctor to deliver a clear picture of your progress and respond to any sudden changes to your health.

How home care can help you or your loved one:

    • Skilled nursing visits and education to help manage a chronic illness
Our care helps your loved one control their symptoms and understand their condition. Our clinicians educate and care for patients with diabetes, COPD, heart failure, Alzheimer’s disease and other diagnoses.
    • Help after a hospital stay
We help your loved one recover and get back on their feet after a hospitalization so they can stay in their home and live as independently as possible.
    • Help managing medications
We help your loved one understand and take their medications at the right time and in the right amounts. We look for medication conflicts and help to clear up any confusion.
    • Focus on fall management to help prevent falls
Our leading balance protocols can help prevent falls and promote safety in the home.

If you or your loved one are covered under Medicare A benefits, our services may be available at no cost to you. We also accept many private payers. To find out more, just call us. We’ll be glad to discuss your situation with you.

    When Home Health Isn’t Enough

Hospice care is specially designed to comfort patients and family members when patients have reached a point in their lives where the goals of care become comfort focused and are no longer about the cure of the illness. Under the direction of a physician, our interdisciplinary team of clinicians offers a special kind of caring that encompasses the medical, social, emotional, and spiritual aspects of treatment.

    Our hospice provides:

    • 24/7 availability of a nurse on call
    • Emotional support and education
    • Volunteers who enrich patients’ lives
    • Hospice aides
    • Coverage under the Medicare benefit for medical equipment, prescriptions and other supplies related to the hospice diagnosis
    • Memory Care for late-stage dementia patients and patients with other late-stage diagnoses accompanied by dementia. We provide specialized clinicians who truly understand the unique symptoms, struggles, and challenges that dementia patients and their families face.

Kindred at Home is a proud member of We Honor Veterans, a program of the National Hospice and Palliative Care Organization and the Veterans Administration, which is designed to empower hospice professionals to meet the unique needs of dying veterans. Learn more at www.wehonorveterans.org.

The Right Care at the Right Time

With home health and hospice together, our clinicians are trained to get your loved one in the right setting. If your loved one is not ready for hospice yet, they may benefit from home health services. You can also contact us without being completely sure what kind of care they need, and know that we have the expertise to guide you.

  • Safe Strides for Home Health

Traditional balance dysfunction programs treat every patient the same way. Conventional programs provide gait training and strengthening exercises. Our home health clinicians recognize that each patient is different, and we work to address the root of the problem first. Then we devise a plan of care to address each patient’s specific needs. The result is a customized program that works because it’s built around you.

  • Orthopedic Care for Home Health

Our home health orthopedic specialty program allows patients to receive innovative rehabilitative care at home, whether for post-operative care, joint replacement or another orthopedic condition.

This unique program promotes independence and ability to function while helping to reduce the risk of unplanned hospital visits. Administered by specially-trained orthopedic therapists and nurses, our orthopedic hands-on program combines patient education and treatment.

  • Low Vision Care for Home Health

Our low vision home health specialty program helps address the causes of vision problems such as macular degeneration, diabetes, glaucoma, stroke and more. The low vision program teaches patients how to make the most of existing vision while helping to compensate for vision loss. We provide you or your loved one with the skills, equipment and support that will help you function at your best.
