Our low vision home health specialty program helps address the causes of vision problems such as macular degeneration, diabetes, glaucoma, stroke and more. The low vision program teaches patients how to make the most of existing vision while helping to compensate for vision loss. We provide you or your loved one with the skills, equipment and support that will help you function at your best.
Our memory care program for home health patients takes a specialized approach toward patients with mild cognitive impairment and various stages of dementia. Our treatment enhances retained abilities and focuses on what patients can do instead of what they might be unable to do. Clinicians focus on improving communication (both verbal and non-verbal). Our care involves the physician and the patient’s family to facilitate support. The goal is to enable each patient to live as independently and safely as possible.
Our home health orthopedic specialty program allows patients to receive innovative rehabilitative care at home, whether for post-operative care, joint replacement or another orthopedic condition.
This unique program promotes independence and ability to function while helping to reduce the risk of unplanned hospital visits. Administered by specially-trained orthopedic therapists and nurses, our orthopedic hands-on program combines patient education and treatment.
Traditional balance dysfunction programs treat every patient the same way. Conventional programs provide gait training and strengthening exercises.
