Our cardiopulmonary program can help patients and their physicians manage heart and lung health at home. Specially trained home health care teams work closely with physicians to provide patients with a wide variety of disease management choices, including early detection and intervention, daily in-home monitoring, thorough evaluation of current medications for possible interactions and side effects and lifestyle adaptation. A wellness journal is used to chart day-to-day progress and identify environmental triggers. A walking program builds strength and mobility.

This unique program promotes independence and ability to function while helping to reduce the risk of unplanned hospital visits. Administered by specially-trained orthopedic therapists and nurses, our orthopedic hands-on program combines patient education and treatment.
Our memory care program for home health patients takes a specialized approach toward patients with mild cognitive impairment and various stages of dementia. Our treatment enhances retained abilities and focuses on what patients can do instead of what they might be unable to do. Clinicians focus on improving communication (both verbal and non-verbal). Our care involves the physician and the patient’s family to facilitate support. The goal is to enable each patient to live as independently and safely as possible.
